• Dementia Care

  Our proprietary program, DementiaWise®, engages and enhances the lives of people with Alzheimer’s disease and other forms of dementia while providing support and education for the family. Caregivers take special care to recognize behavioral triggers and use redirection, which may help prevent or reduce any challenging dementia behaviors, while helping clients remain independent for as long as possible.

In addition, our agency can perform fall-risk assessments in your loved one’s home and can provide fall prevention tips. ComForCare can help transfer your loved one from a medical facility such as a hospital to an assisted living community or to their home. Short-Term Assistance With hospital-to-home transition care, our caregivers are able to provide transportation assistance and help an individual with meal preparation, running errands, following discharge plans, light housekeeping and personal hygiene.
